Entry Information
There are 2 ways to complete the Student Entry Form:
- Open the PDF in your browser and go to File > Print to print the blank form. Fill the form out, make sure it's signed, and attach it to your child's artwork.
- Open the PDF in your browser and fill it out online. When finished, go to File > Print to print out the completed form. Sign the form, have your child do the same, then attach it to your child's artwork.
For best results in viewing or printing the form, use the free Adobe Acrobat Reader.

Important Note:
Please read the Student Entry Form carefully, fill out ALL sections, and sign it. We cannot accept incomplete or unsigned forms. All submission envelopes must include child's name, his/her teacher's name, and art category. Artwork should be dropped of in the Impressions collection box outside the school's front doors between 7AM - 9AM by the deadline date.
Be sure to check the box on the entry form in order to have your artwork displayed online and judged.
